This unit describes the outcomes required for those who assist individuals with younger onset dementia to maintain independence, live well with dementia, and plan for the future.

Elements and Performance Criteria
Analyse care needs of people living with younger onset dementia
- 1.1 Determine and review common models of care for living with younger onset dementia
- 1.2 Determine key challenges facing people living with younger onset dementia
- 1.3 Determine current and future needs of people living with younger onset dementia
- 1.4 Research community services applicable to the needs of people living with younger onset dementia
Develop and maintain co-operative working relationships
- 2.1 Build and maintain relationships with other service providers and networks offering services applicable to the needs of people living with younger onset dementia
- 2.2 Consult with people living with younger onset dementia and other stakeholders to establish service requirements
- 2.3 Promote the unique needs of people living with younger onset dementia and other stakeholders, to networks and service providers
Review services for people living with younger onset dementia
- 3.1 Review organisational documentation available for the person living with younger onset dementia to determine individual needs
- 3.2 Consult with the person living with younger onset dementia and other stakeholders to establish the individual’s needs
