Description
This unit describes the performance outcomes, skills and knowledge required to provide nursing care to a person with diabetes including assessing needs, planning and implementing nursing interventions, evaluating outcomes, and educating the person on the condition and available resources.This unit applies to enrolled nursing work carried out in consultation and collaboration with registered nurses, and under supervisory arrangements aligned to the Nursing and Midwifery Board of Australia (NMBA) regulatory authority legislative requirements.The skills in this unit must be applied in accordance with Commonwealth and State/Territory legislation, Australian Standards and industry codes of practice.No occupational licensing, certification or specific legislative requirements apply to this unit at the time of publication.

Elements and Performance Criteria
1. Assess needs of a person with diabetes.
  • 1.1. Apply requirements of the pathophysiology of types of diabetes.
  • 1.2. Perform nursing assessment using contemporary assessment tools, resources and strategies and incorporating review of health status, lifestyle and treatment regimes.
  • 1.3. Determine understanding of condition, self-management strategies and medications.
  • 1.4. Identify possible factors impacting the health and significant alterations in the condition.
  • 1.5. Identify family and carer understanding of and involvement in the person’s diabetes care and the impact on care planning and provision.
2. Perform nursing interventions to assist in achieving and maintaining optimal diabetes health.
  • 2.1. Prioritise care according to needs and re-prioritise care activities when circumstances change.
  • 2.2. Apply plan of care and ensure it reflects the care needs of a person with diabetes.
  • 2.3. Identify response to nursing interventions and understanding of ongoing management of the condition, including strategies for self-management.
  • 2.4. Evaluate blood and urine test results related to diabetic condition and report findings to the multidisciplinary health care team.
  • 2.5. Administer prescribed and emergency medicine as indicated.
  • 2.6. Liaise with registered nurses about alterations in condition while providing ongoing support.
3. Identify diabetes care services in the Australian health care environment.
  • 3.1. specialist services and complementary roles of organisations and individuals involved in supporting and delivering diabetes care.
  • 3.2. Identify need to refer person to a diabetes registered nurse educator and other specialist services and complementary roles of organisations and individuals involved in supporting and delivering diabetes care.
4. Evaluate care plan for a person with diabetes and support self-management.
  • 4.1. Identify and evaluate diabetes management plan to ensure compliance and escalate intervention where identified.
  • 4.2. Evaluate nursing interventions and identified outcomes against evidence-based best practice in diabetes nursing care.
  • 4.3. Evaluate person’s understanding of the diabetes condition, medications, therapeutic regimes and self-management.
  • 4.4. Promote self-management of condition and assist person to provide accurate information to families and carers on diabetes care needs.
  • 4.5. Document uptake of health promotion initiatives to support self-management.
